The Case Manager/ QIDP is responsible for managing a caseload of clients; developing client-centered Individual Plans, and Implementation Strategies, including health related and behavior plans and activities; training and monitoring DSPs and Rehabilitation Managers to assure correct implementation and documentation; and maintaining client records per policies and procedures. The Case Manager/QIDP is also familiar with and maintains all applicable program and agency rules and applicable DHS, DPH and CARF standards. The Community Day Services program facilitates services for individuals from diverse backgrounds, embraces differences in individuals and promotes a positive attitude with peers and community members.
- Bachelor's Degree in a Human Service Area.
- One year of experience, working directly with individuals with intellectual disabilities and/or other developmental disabilities.
