What will you be doing?

• Providing counseling and case management services to a caseload of approximately 30 – 50 clients with diagnoses of persistent mental illness and/or substance abuse
• Developing and implementing individual programs to maximize the clients’ independent community living skills
• Completing daily documentation of mental health services provided to clients
• Working with a team of behavioral health service providers within the agency to ensure a positive client-centered approach

• Bachelor’s degree in human services
• Experience working in social services (required)
• A valid Illinois driver’s license and automobile insurance
