Certificates of Origin
A Certificate of Origin requires the authorized signature of the local Chamber of Commerce designated representative and the seal of that organization. Countries require forms to be notarized as well. It is requested that companies call ahead (847-741-5663) to make an appointment with the Chamber staff authorized to issue Certificates of Origin.

Through a network of trade specialist located in Chicago and 7 well established regional foreign offices, OTI provides businesses with lead generation support and connects companies to foreign governments, chambers of commerce and other strategic partners around the world.
